Verify need from brigade leads for self-service form for members

We want to qualify the need as to whether brigade leads want a self-service version of the survey for wider brigade members to understand what they would prioritize and any effective processes and practices they want to share.

Dependencies

• Conducting enough interviews to determine interest is present

Action Items

• Add to the google form a question asking whether self-service version of the form is required (UXR to ask the question)
• Based on confirmation of above, create a self-service version
• Customize the form for Brigade Members
• Review the form changes 29th Dec
• Rename title of form
• Capture feedback from self-service forms
