This is usually a case of improper configuration where the mediaAvailability sync job thinks a series/movie is removed due to a misconnection/misconfiguration and it removes the request. So the request will no longer exist. Its not really a bug (if that is whats happening to you. Without proper logs I cannot help. Will need logs from media availability sync job. Youll need to parse through older logs and find all the media availability logs and find which one removed it)
