Currently in AEV we cannot view "Second Level" file attachments. These appear within the PDF as:

There is an alternate Aspose method for getting attachments, using https://apireference.aspose.com/pdf/java/com.aspose.pdf.facades/PdfExtractor, which seems to pick these up
