Created CHANGES.362 for DCMTK release 3.6.2.

Created CHANGES.362 for DCMTK release based on CHANGES.361 and the commit history since the last snapshot. Modified CHANGES to no longer only refer to DCMTK snapshots. CHANGES.361 has been removed since the content is included in CHANGES.362.
