internal: Include revisions of multiple bundles in decision log event

Earlier the decision log events generated by the plugin did not set the
revision in cases where multiple bundles were configured. This change
includes these revisions in the log event and also adds a deprecation label
to the "revision" field of the eval result.

.github: Update post tag workflow to use env files

The post tag workflow used the `set-env` command to
set the TAG_NAME env variable. The `set-env` command is
now disabled. This change updates the worklow to remove
usage of the `set-env` command to use environment files
instead.
